Surgical repair of gastroschisis at birth. A systematic review

Abstract

Objective: to evaluate the management and surgical techniques for the management of gastroschisis in the newborn. Methods: Eligibility; a search was conducted with the PRISMA 2020 declaration for a systematic review on the management of gastroschisis, inclusion and exclusion criteria were developed on the subject of research, articles that met the inclusion criteria were selected for further analysis together; scientific medical articles were collected from databases such as Pubmed, ELSEVIER, Guidelines from the European Reference Network for Rare Inherited Congenital Anomalies (ERNICA), on the treatment of gastroschisis and information obtained from journals such as NEJM, PMC; the risk of bias from individual studies, depended on statistical heterogeneity and synthesis of results through analysis. Results: Included with a total of 36 scientific articles, with participants of population studies; summary of results: with the implementation of surgical treatment, neonatal mortality has been reduced. The primary closure by simil exit technique is performed in cases of gastroschisis whose defects are <5cm, with an intestinal diameter greater than 18 mm, after 30 weeks of gestation, by elective caesarean section, scheduled. Secondary closure with the Bianchi technique is another surgical approach used to treat gastroschisis and is performed when dealing with complex gastroschisis or when the defect diameter is greater than 5 cm and the intestinal diameter greater than 18 mm with intestinal distress. Discussion: Limitations of the evidence. Limitations depend on statistical heterogeneity. Interpreting: This research assesses the great importance of neonatal surgery in the management of gastroschisis both techniques had good results with their respective indications.
